In order for us to determine if you are a good candidate for the "Hair Stroke" procedure we will need some information.
*Ever had permanent makeup/microblading on your eyebrows before
(We do not work on anyone with previous permanent makeup on their
*Pregnant, nursing, or trying to get pregnant
*Any scars or experienced any head trauma in the brow area
*Heart Conditions/Pace Maker/Defibrillator/Defibrillation (No exceptions)
If you are any of these medications, you will bleed and the pigment WILL NOT retain)
*Cirrhosis of the Liver
(Susceptable to infection)
*Surgical Forehead/Brow lift
(Scar tissue will prevent proper healing)
(Hair loss due to auto immune disease)
(Hair strokes will appear more blurred or blended)
*Trichotillomania(Compulsive pulling of body hair)
(Due to constant pulling, scar tissue is prominent and pigment will not heal properly)
*Shingles-Have you EVER had shingles on your face
(No exceptions...PMU will not be performed as the procedure could cause a flare up)
* Eczema, Psoriasis or Dermatitis in or around the brow area
(Constant flaking/itching/irritation/shedding of skin)
*Oily, Combinaiton or Severely oily skin
(The hair strokes will appear more blended, solid or not retain at all)
*Large pores on your forehead & in the brow area
(Pigment will blur/blend in large pores looking powdered)
If you have hot flashes during the procedure, the pigment will not retain and Mary may have to stop.
(You will be required to sign and addendum prior to procedure)
*If your body runs hot
* If you are under the age of 25, you will only be able to do this procedure a few times(1-5). The younger you start, the younger you are when it can no longer be done. It is recommended to do when you are older so that you can maintain for a few years.
*Platelet Disorders-Aggregation Disorders
An aggregation disorder is when platelets do not bind with fibrinogen and other proteins in order to stick to other platelets. As a result the platelets cannot form a plug to stop the bleeding from a damaged blood vessel.
*Moles/raised areas in or around the brow area
(Pigment will not be put into anything raised.)
*Piercing in the brow area
(Scars can cause the pigment to migrate)
*Deep wrinkles in the brow area
(The Hair Strokes will not lay properly in the creases, giving the brow an uneven look)
*Hair transplant for your eyebrows
(Pigment will not take in the scar tissue where the plugs were placed)
*Thyroid condition and taking medication for this condition
(Hypo, Hyper Susceptible, Graves Disease, Hashimotos)
*Rosacea (severe reddening of the face)
*Fitzpatrick Skin types 1 & 2 (red heads/white skin/light eyes, Strawberry blondes and some blondes)
Due to hypersensitivity the skin does not take or heal the pigment well.
(Addendum is required if proceeding)
*On Accutane (acne medicine)or any other prescribed acne medicines within the last year (Dr's note required with date of last use)
*Any type of Organ Transplant or Anti-Rejection Medications
*Auto Immune Disorder of any kind (MS, RA, Lupus or the like)
(Due to the medicines to treat these diseases, pigment will not retain)
*List medicines currently taking including all vitamins.
(NO FISH OIL 2 WEEKS PRIOR)
*MRSA (can be very contagious)
* Prone to keloiding or hypertrophic scarring
*I Do NOT do super thin (pencil thin) eyebrows as hair strokes will not work.
*Extremely Thin skin
(Transparent or Translucent or very vascular)
*Planning on any type of facial surgery in or around the area to be treated. Must be min. 1-6 months depending of surgery performed.
*Frequent Exercise (4-7 days per week)(Addendum required)
(Due to the frequent production of sweat(salt), the pigment WILL NOT retain, fade very quickly, appear blurred or change in color)

THESE CONTRAINDICATIONS CAN ALSO PRESENT LATER IN LIFE. FUTURE PROCEDURES MAY NOT BE PERFORMED AND/OR AN ADDENDUM WILL BE REQUIRED.

Please keep in mind that even if an appointment is scheduled, there is the possibility that an unforeseen issue that was not identified in the photo may prevent the procedure from being performed.
If you show up and the appointment is canceled, you will forfeit your $50 deposit and will be required to secure a new appointment with another deposit.
If you work out regularly (4-7 days a week), your eyebrows WILL fade very fast due to the production of salt/sweat. Salt is used to remove permanent makeup. Appointments will not be moved earlier if you have premature fading due to lifestyle. They WILL appear more powdered or blurred under the skin as well. If you are not ok with this, then you should not have the procedure done. Mary reserves the right to refuse future services if she feels the results wont be to the clients satisfaction. Addendum will be required.
YOU CANNOT HAVE A TAN 30 DAYS BEFORE & ABSOLUTELY NO SUN 30 BEFORE AND 30 DAYS AFTER (This means you cannot go out in sun with a hat and glasses on, this means no SUN or HEAT FOR 30 days AFTER at all. If you resume tanning, the results will fade quickly, heal darker, fade super fast or not retain at all. Mary will not see you any sooner than your routine scheduled appt. If you ruin the results by going out in the sun too soon, Mary may not be able to work on you in the future.

This is an investment in your face and Mary takes that very seriously.
